I am ending my STEM OPT early:

• I have permanently departed the US, and I need to end my STEM OPT early.
• I have been admitted to a new program of study in the US, and wish to transfer my F-1 status to another institution to begin my new program (or return for a new program at Harvard). I will contact my HIO Advisor to coordinate the transfer of my F-1 record so I can begin my new degree program.
• I have changed my immigration status in the US, and I need to end my STEM OPT.  I will be required to upload documentation of my new immigration status.

A completed Final Evaluation on Student Progress is required (bottom half of page 5 of the I-983).
