The link referenced below is for students and scholars to register with the HIO upon initial arrival, after a change of immigration status, or after a return from a leave of absence. Complete the HIO Registration Form only after you arrive in the U.S.

Please have the following available as electronic files when completing the registration form:

• Passport (ID page and visa)
• I-20 or DS-2019 form
• I-94 record

There may be additional documentation you need to provide pertaining to your non-immigrant status. Please note, all J-1 Student Interns must submit their completed and signed DS-7002 form.

Please note that if you previously received instructions to complete a PDF HIO Registration Form to send by email, this new process replaces this and an email after completing the form is not necessary.
