STEM OPT Reporting/ Change in Employment
I have had a change in my employment situation since I last submitted a Form I-983 to the HIO. A change may include:

My STEM OPT has been APPROVED and I am reporting a change of employer.
- A completed Final Evaluation on Student Progress (bottom half of page 5 of the I-983) is required for the employer from which you are departing, as well as a new, completed I-983 for the new employer.

My STEM OPT application is PENDING with USCIS, and I am reporting a change of employer.

A completed Final Evaluation on Student Progress (bottom half of page 5 of the I-983) is required for the employer from which you are departing, as well as a new, completed I-983 for the new employer.

There have been material changes to my I-983 (e.g., change in supervisor, change in job title or duties, change in location, change in salary, etc.).

A new, completed I-983 reflecting the material change(s) is required.

I am reporting that I have a new employer and/or a concurrent/additional employer.

A new, completed I-983 from the new/concurrent employer is required.

I am ending my STEM OPT employment with an employer and submitting this form within 10 days of my last day with the employer. This employment ends before my 24-month STEM OPT period ends.

I have not exceeded my unemployment days and I am currently job searching.

I still plan to use my F-1 STEM OPT status to work in U.S.

I have not changed U.S. immigration status or plan to permanently depart the U.S. prior to the end of my F-1 STEM OPT status.

A completed Final Evaluation on Student Progress is required (bottom half of page 5 of the I-983).
